Registration Information:

Please register 2 business days prior to your child’s first day in the program

Confirmation of Registration is required– confirmation will be sent via Email

Information Required for Registration:
- Emergency Contacts: names, addresses and phone numbers
- Persons To Whom Child May Be Released: other than parents- names, addresses and phone numbers
- Health Insurance Provider AND Policy Number: Contact Children’s Health Insurance Program (CHIP) at Medicaid.gov if your child does not have insurance
- Primary Physician Information: address and phone number
- Auto Pay: to authorize monthly credit card usage, go to Auto Pay within your Parent Portal
- Have a Credit Card or Bank Check readily available- If you do not have a credit card/bank check, you must come into our main office
Registration Fees- registration cannot be completed without full payment of fee
- $100 per child
- $150 per family (multiple children)
- ELRC: $50 per child, $75 per family
