Registration for New Families

Registration Information:

  • Please register 2 business days prior to your child’s first day in the program

  • Confirmation of Registration is required– confirmation will be sent via Email

  • Information Required for Registration:

    • Emergency Contacts: names, addresses and phone numbers
    • Persons To Whom Child May Be Released: other than parents-  names, addresses and phone numbers
    • Health Insurance Provider AND Policy Number: Contact Children’s Health Insurance Program (CHIP) at Medicaid.gov if your child does not have insurance
    • Primary Physician Information: address and phone number
    • Auto Pay: to authorize monthly credit card usage, go to Auto Pay within your Parent Portal
    • Have a Credit Card or Bank Check readily available-  If you do not have a credit card/bank check, you must come into our main office

Registration Fees- registration cannot be completed without full  payment of  fee

  • $100 per child
  • $150 per family (multiple children)
  • ELRC: $50 per child, $75 per family
